THIS CONTRACT IS BETWEEN TOP DOG PET RESORT & DAYCARE AND THE PET OWNER
WHOSE SIGNATURE APPEARS BELOW.
1. Owner agrees to pay to kennel all cost for boarding & special services requested for the pet during the time it is in the care of the kennel as posted in kennel office & indicated on this contract, & pay all (if any should arise) veterinary cost for the pet during the time pet is in the care of the kennel. Owner further agrees the pet will not leave the kennel until all charges are paid.
2. Top Dog Pet Resort & Daycare agrees to exercise reasonable care for the pet, and to keep its premises sanitary and properly enclosed. It is expressly agreed that the kennel’s liability shall in no event exceed the lesser of the current chattel value of a pet of the same species or the sum of $200.00 per animal boarded. The owner of the pet agrees to be solely responsible for any acts or behavior of said pet while it is in the care of the kennel.
3. Top Dog Pet Resort & Daycare does not assume and shall not be held responsible for any liability with respect to the pet listed in this agreement, of any kind whatsoever, arising out of or from boarding of this pet, or any damages which may accrue from any other cause whatsoever, including loss by fire, theft, disease, running away, death, injury to persons, animals, or property, or death or injury to any other animal caused by the within named pet.
4. By signing this contract owner certifies to the accuracy of all information given about said pet and specifically represents that he or she is the sole owner of the pet, free & clear of all liens & encumbrances.
5. Owner specifically represents that the pet has not been exposed to distemper, parvo, rabies, or kennel cough within the past 30 days.
6. Owner understands that the kennel cannot be responsible for lost, dirty, damaged, or destroyed belongings.
7. If pet becomes ill or if the animals health requires professional attention, the kennel, in its sole discretion, may engage the services of a veterinarian and the expenses thereof shall be paid by the owner of the pet.
Notice: Any pet with fleas or ticks upon arrival will be treated at the owner’s expense prior to entering the kennel area. In addition the kennel reserves the right to refuse services at any time for any reason regardless of reservations or prior arrangements.
